What You’ll Be Doing As a Care Planner, you’ll take ownership of planning and managing care services to ensure high-quality outcomes for clients. Your responsibilities include: Coordinating care services across NHS Trusts, private hospitals, social services, community packages, and other healthcare settings Working closely with clients, families, and healthcare professionals to assess needs and develop personalised care plans Ensuring timely and efficient service delivery, often at short notice Maintaining accurate records, ensuring compliance with healthcare regulations and company policies Communicating effectively with multidisciplinary teams Monitoring and reviewing care plans to maintain high standards and continuity of care Supporting staff allocation, matching the right professionals to client needs Responding professionally to any concerns, queries, or urgent changes Participating in the on-call rota: 1 night per week and 1 shared weekend per month Every decision you make helps clients receive the best possible care. What You’ll Bring We’re looking for someone organised, confident, and proactive.
